Job Description
- Monitoring & Reporting of Medication Errors
- Detection & Reporting of adverse drug reactions
- Conducting research to aid drug discovery and development
- Monitoring antibiotic usage in the hospital, sticking to antibiotic guidelines, development of antibiograms, antibiotic restrictions forms & conducting point prevalence audits
- Monitoring & helping in the compliance of AMS SOP & compliance to ICMR, WHO. CDC & other manuals
- Helping with hospital formulary
- Identifying and separating LASA & high alert drugs
- Regular monitoring usage, storage and compliance to the regulatory components of Narcotics substances and drugs
- Preparing brochers for spreading awareness about various medications
- Patient counselling
- Medication reconciliation
- Monitoring Drug – Drug interaction & Drug Food interaction
- Monitoring ADRs
- Monitoring ADEs
- Monitoring Anti biotic Usage, DOTS & LOTS
- Monitoring Nephrotoxic Drug & Hepatoxic Drug
- Active participation and conduct regular PTC meeting & Compliance to NABH, NABL, AACI
- Prioritize level of care based on critical needs
- Prevent, detect and resolve medication – related problems nd to make appropriate evidence-based, patient centered medication therapy recommendations as part of the inter
- Data tracking and data collection for Antimicrobial Stewardship Programme
