Job Description
  • Monitoring & Reporting of Medication Errors
  • Detection & Reporting of adverse drug reactions
  • Conducting research to aid drug discovery and development
  • Monitoring antibiotic usage in the hospital, sticking to antibiotic guidelines, development of antibiograms, antibiotic restrictions forms & conducting point prevalence audits
  • Monitoring & helping in the compliance of AMS SOP & compliance to ICMR, WHO. CDC & other manuals
  • Helping with hospital formulary
  • Identifying and separating LASA & high alert drugs
  • Regular monitoring usage, storage and compliance to the regulatory components of Narcotics substances and drugs
  • Preparing brochers for spreading awareness about various medications
  • Patient counselling
  • Medication reconciliation
  • Monitoring Drug – Drug interaction & Drug Food interaction
  • Monitoring ADRs
  • Monitoring ADEs
  • Monitoring Anti biotic Usage, DOTS & LOTS
  • Monitoring Nephrotoxic Drug & Hepatoxic Drug
  • Active participation and conduct regular PTC meeting & Compliance to NABH, NABL, AACI
  • Prioritize level of care based on critical needs
  • Prevent, detect and resolve medication – related problems nd to make appropriate evidence-based, patient centered medication therapy recommendations as part of the inter
  • Data tracking and data collection for Antimicrobial Stewardship Programme
